How to Fill In PDF Applications
To fill out these applications on your computer, click on link to the application form. You will see the application form open in a new window. Click the “download” icon [ ] you see on the upper right of the window with the application. It will download a PDF to the Downloads folder of your computer. Double-click the file, open it in Acrobat Reader, and click the button on the right side that says Fill & Sign. Type into the application form fields with your computer keyboard. Please print out the form and mail it to the address listed.
