2021-2022 Forms Appeal and Release Forms
Please only submit requested forms. The Financial Aid office will only accept documents if all required documents are submitted at the same time. Please review your To Do List in Bay Navigator before submitting Financial Aid documents. If we have requested additional forms, you will receive an email or letter to your MassBay email account indicating which forms we need from you.
You may request a reconsideration of your financial aid through the process known as Professional Judgement.
We recognize that a family’s income is not always consistent from one year to the next, and that circumstances beyond a family’s control may inhibit the ability to contribute to educational expenses. Therefore, we welcome the opportunity to discuss any change in your family’s financial status that is not reflected on the FAFSA.

Submit your forms by faxing them to us at 781-239-2607. We accept faxes 24 hours a day. We do not recommend forms be sent to us via an email attachment as email is not a private or confidential method of communication. Lastly, you may mail your form via U.S. Mail to the following address:
